You have been invited to book your annual heart and/or kidney review with an advanced clinical pharmacist.
You have received this invitation because you fall onto one or more of the following chronic disease registers:
- Atrial fibrillation (AF)
- Heart failure (HF)
- Chronic Kidney Disease (CKD) – Stages 4 and 5
What happens in my appointment?
The pharmacist will talk to you about your symptoms and how you are managing with your condition.
You will have basic observations taken (e.g., blood pressure, pulse, body mass index), be sent for further investigations (e.g., blood tests) and a discussion around your diet and lifestyle.
You may also have a discussion around any new or current medications and treatment plans.
This is a 15 minute review with an advanced clinical pharmacist.
