We do not allow prescription requests over the phone.
Our traditional ordering methods are:
- Online services (such as the NHS app).
- Completing a “repeat request form” within the surgery.
- Posting your repeat prescription request to: Bloomfield Medical Centre, 118-120 Bloomfield Road, Blackpool FY1 6JW.
If you are unable to use the above ordering methods due to vulnerabilities or health conditions, you can ask your pharmacy to order your prescriptions on your behalf. You must inform us of this agreement which we will check with the pharmacy.
