JOB DESCRIPTION

JOB TITLE: Health Care Assistant

RESPONSIBLE TO: Lead Nurse/Nurse Practitioner

ACCOUNTABLE TO: Clinical Services Manager

GRADE: Agenda for Change Band 2/3

Role of The Service

GP Led Health Centre is a new open access service based in the Healthcare Centre at Whitegate Drive. This service will be co-located along side and sharing the reception with NHS Blackpool’s provider service: the Walk-in-Centre. The GP Led Health Centre will be open every day of the year from 8am to 8pm. This will be the main base of work however on occasions you may be required to work at the Urgent Care Centre or Bloomfield Medical Centre.

Job Summary

To provide support to the qualified nurses whole health care team, providing a range of clinical duties, personal care to patients, reporting changes to in patient’s condition to qualified staff. The post holder will also contribute to administrative, clinical, housekeeping duties within the practice locations.

Duties And Responsibilities

  • Post holder will ensure prompt reporting of any relevant information regarding patients including observation of progress or deterioration in the patient’s condition.

  • Endeavour to maintain good standards of communication both in written and oral form, within the multi-disciplinary team.

  • Make entries on patient charts to record clinical interventions.

  • Complete associated documents to accompany specimens.

  • Communicates with patients and carers in a polite and pleasant manner.

  • Receive and deliver patient related information to and from other wards and departments via the telephone.

  • Acknowledge and accommodate barriers to communication and understanding including speech, hearing, language and emotion.

  • May be required to encourage patients to comply with programme of care.

  • Ensure patient well-being by accurately monitoring, recording and reporting variations in vital signs of patients in their care to the qualified Nurse.

  • Initiate emergency action when required

  • Obtain and dispatch specimens on request.

  • Record patient observations.

  • Assist and support clients within the GP Led Health Centre as necessary.

  • Undertake routine clerical duties eg. answering the telephone, taking messages, and filing, photocopying, and dealing with queries.

  • Assist in the ordering and supplying of materials and equipment eg. stores after authorisation by the Clinical Service Manager

  • Inform the Clinical Service Manager of any untoward incidents eg. falls, verbal aggression, criminal damage.

Education And In Service Training

  • Be fully conversant in the clinical computer system used within the GP Led Healthcare Centre/Walk-in-Centre

  • Attend relevant study/induction days, seminars, courses etc for individual development and for the benefit of the service.

  • Undertake additional responsibilities as directed by Clinical Service Manager/Administrative Manager eg. Computer system supervisor/trainer.

The above list of duties and responsibilities is not intended to be fully comprehensive and may be amended to take account of changing circumstances or requirements following consultation with the post holder.

Where necessary relevant training in the operation of new or unfamiliar equipment, software or procedures will be provided or arranged.

Working Relationships

Confidentiality

In the course of your duties you may have access to confidential information about patients, staff or health service business. On no account must such information be divulged to anyone who is not authorised to receive it. Confidentiality of information must be preserved at all times whether at or away from work. The Practice has in place a ‘Whistleblowers Policy’ for staff wishing to express concerns.

Data Protection Act, 1998

Carry out any requirements within the duties applicable to the Data Protection Act, 1998.

Health And Safety At Work Act, 1974

Observe all responsibilities and carry out all duties, whether general to all employees or specific to the post, relating to Health and Safety in accordance with the Practice and Departmental Safety Policies and any statutory requirements.

Clinical & Corporate Governance/Quality Assurance

All employees are expected to comply with the clinical and corporate governance arrangements of the Primary Care Trust and the Practice. Every employee is personally responsible for the quality of the work, and standard of care, which they individually provide. It is their duty to seek to attain the highest standards achievable both individually and collectively within their knowledge, skills and resources available to them.

The PCT manages all research in accordance with the requirements of the Research Governance Framework. As an employee of the Practice you must comply with all reporting, systems and duties of action put in place by the PCT to deliver research governance.

Any employee wishing to undertake research within the Trust must obtain both Practice and Primary Care Trust approval prior to commencement of any research.

Outside Employment/Outside Interests

If you have other work or outside interests, this must not conflict with your duties and responsibilities of you attendance for work as an employee of the Practice.

It is a condition of appointment that you must inform your manager before taking up of any private practice, work for outside agencies or other employers, other work for this Practice (including bank work), voluntary work or outside interests you have or propose to have. This is to ensure there is no question of it creating a ‘conflict of interest’ with your NHS duties. You must also therefore seek your manager's approval before taking on any such other work or outside interest at any time after entering employment.

Working Time Directive

You are required to comply with the regulations governing working time and to any locally agreed associated arrangements.

Healthcare Assistant

Essential Desirable Qualifications

  • Good standard of education or experience

  • NVQ Level 2

 

Experience

  • Previous health care work experience

  • Experience of dealing with the general public

  • Previous Primary care experience

 

Knowledge & Skills

  • Ability to work as a team member

  • Able to exercise initiative in handling matters

  • Organisation and prioritising skills

  • Good communication skills both written and oral

  • Good Interpersonal skills

  • Basic IT skills

  • Venepuncture

Terms and Conditions

The terms and conditions for the post are those set out in the Agenda for Change: NHS Terms and conditions of Service Handbook. Practice Policies and Procedures will also apply to your contract of employment.

Duration Of Employment

The appointment is permanent.

Hours Of Duty

Hours of duty for the post are part time to be agreed, to be worked over 7 days (to include evening and weekend work), to the needs of the service.

Salary

The basic rate of pay for full-time working in this post will be within the Agenda for Change Pay Band 2/3 which is currently £13,233 per annum rising to a maximum of £18,157 per annum. Pay progression is subject to satisfactory performance and demonstration of the agreed knowledge and skills appropriate to that part of the pay band or range. All pay bands include Foundation and Second Gateways where assessment of the application of knowledge and skills necessary for pay progression will be made.

Annual Leave

27 days annual leave increasing to 29 days after 5 years and 33 days after 10 years service. Part-time entitlement is pro-rata to full-time.

Public Holidays

Full-time staff are entitled to each Public Holiday that falls throughout the year (part-time staff are entitled to a proportionate number of Public Holiday hours based on their basic weekly contracted hours).

Sick Pay Scheme

The Agenda for Change: NHS Terms and Conditions of Service Handbook are in operation and is dependent upon length of service.

Pension Scheme

The post is superannuable, unless you opt out of the National Health Service Scheme or are ineligible to join. The employee’s contribution is currently 6% and the employer’s is 14%.

Travel

Staff who use their privately owned motor vehicle on official Practice business use, are required to maintain a policy of insurance extending to the use of the vehicle for official business purposes.

Travel expenses will be paid in line with the Agenda for Change: NHS Terms and Conditions of Service Handbook.

Notice

Termination of employment is subject to 4 weeks’ notice in writing from the employee. The Practice shall be required to give such notice of termination to the employee as is in accordance with the terms and conditions. (This will vary with length of service).
