Your Information - How We Use It
Healthcare professionals caring for you have to keep records about your health and any treatment you receive from the NHS to help with continuity of your care. These may be written down or held on computer.
Information may include:

Name, address, date of birth, NHS number and next of kin.

Contact we have had with you - eg practice visits.

Notes and reports about your health and any treatment you need or have received.

Results of investigations - eg xray and laboratory tests.

Relevant information from other healthcare professionals, relatives or those who care for you and know you well.
Information about your treatment may be used to help us:

Plan our services for the future.

Monitor our performance and ensure that we meet the healthcare needs of the local population we serve.

Review the care we provide to ensure it is of the highest standard.

Investigate complaints, legal claims or untoward incidents to improve the services we deliver.
We will not disclose your information to third parties without your permission, unless there are exceptional circumstances - eg where the law requires this.
If you have any concerns about the way we use your information or you wish to access your own records please ask for the practice manager who will be happy to discuss the matter with you.
Freedom Of Information - Publication Scheme
The Freedom of Information Act 2000 obliges the practice to produce a Publication Scheme. A Publication Scheme is a guide to the ‘classes’ of information the practice intends to routinely make available.
This scheme is available from reception.
Patient Behaviour
The practice operates a Zero Tolerance approach to the behaviour of patients in a move to protect staff and other patients from violence and/or abuse. This means that any display of abuse or violence, be it mental or physical, may well result in the removal of that person from the practice list. Reception and surrounding areas are covered by closed circuit television and record 24 hours a day.
Disabled Access
The practice is fully compliant with regulations for access for the disabled and benefits from provision of a lift to the upper floor.
Citizens Advice Bureau
Sessions are held at the surgery on a Monday, Tuesday and Wednesday afternoon between 2.00 and 4.00pm. Help and advice is available on issues such as benefits, debt, employment and housing as well as general advice on any subject. Appointments can be arranged by telephoning 0844 477 8582.

Walk-in Centre
The local walk-in centre is situated at 26 Talbot Road. Its opening hours are as follows:
• Monday - Saturday, 8.00am to 9.00pm
• Sunday and Bank Holidays: 9.00am to 8.00pm
